Accounts for pricing sent via tape or email. We will accept a comma delimited file on any of the following mediums: 4mm data cartridge tape, 8mm exabyte data cartridge tape, 100MB Zip drive, 3480/3490 IBM data cartridge tape, CD, 3.5 inch floppy, 1/4" 6250 cartridge tape.

• Facility Name and Address
• CPT Codes, HCFA-UB92 forms
• Primary, Secondary and Tertiary Insurance, If Available
